Tailored Care Management (TCM) is a new program, offered by HealthKeeperz,
designed to support individuals with Medicaid who are living with mental health and/or
behavioral health challenges. It offers each person their own care manager who helps
with various aspects of their healthcare, including doctor's visits, housing, and
transportation.

TCM ensures that everyone's health needs, especially those with
conditions like serious mental illness or developmental disabilities, are properly
addressed.

TCM providers are available to answer questions and ensure smooth
healthcare experiences for all individuals involved.
